Description

Sitagliptin phosphate 100 mg tablet (expressed as sitagliptin). Sitagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor. It prevents breakdown of the gut hormones GLP-1 and GIP, so their levels stay higher after meals. This increases insulin release from the pancreas when blood sugar is raised and reduces glucagon output, lowering both fasting and after-meal glucose. The effect depends on blood glucose, so the risk of low blood sugar is low when used alone and body weight is unchanged. A large outcome trial in patients with heart disease showed no increase in major cardiovascular events or heart failure hospitalisation. Taken with or without food; 100 mg is the usual dose.

Side Effects

Upper respiratory infection, nasopharyngitis, headache; low blood sugar when used with a sulphonylurea or insulin., Less common: joint pain, constipation, nausea, swelling of hands or feet., Rare: acute pancreatitis (severe, persistent abdominal pain), bullous pemphigoid, serious allergic reactions including angioedema and Stevens-Johnson syndrome., It may slightly raise digoxin levels., Not for use in type 1 diabetes, diabetic ketoacidosis, or in patients with a history of serious hypersensitivity to sitagliptin; caution in a history of pancreatitis.

Indication

Type 2 diabetes mellitus in adults, as an adjunct to diet and exercise, alone or with metformin, sulphonylureas, thiazolidinediones, SGLT2 inhibitors or insulin
