Cough, cold and allergy is the segment that fills a GP's waiting room from October to February and again through the monsoon. Acute respiratory infection is among the most common reasons for an outpatient visit in India, cough and cold preparations are one of the largest categories in Indian retail pharma, and montelukast-levocetirizine has become a year-round prescription for allergic rhinitis and asthma. It is a high-frequency, low-ticket segment: many strips and bottles, reordered often, sold by every chemist.
Two things make it a distributor's segment. First, the same molecule set repeats across every prescriber – GP, paediatrician, ENT surgeon, chest physician – so one visual aid covers the whole town. The combinations – ambroxol-terbutaline-guaiphenesin, levosalbutamol-ambroxol-guaiphenesin, dextromethorphan-phenylephrine-chlorpheniramine, paracetamol-phenylephrine cold tablets, montelukast-levocetirizine, montelukast-fexofenadine, bilastine, doxofylline-montelukast – are priced normally. Most cough syrups come in two packs: with a mono-carton, and without (WOMC) at a lower MRP. The formula, bottle and batch are the same. Prescription-driven markets usually take the carton pack; chemist-counter and rural markets prefer the lower-MRP pack. You can stock either or both. In December 2023 CDSCO directed that chlorpheniramine-phenylephrine fixed-dose combinations must not be used in children under four years and that the label must say so. That directive applies to the paediatric cold drops, syrups and suspensions in this range; the prescribing decision is the doctor's. Levosalbutamol-ambroxol junior syrups, levocetirizine-montelukast kid tablets and syrup, and fexofenadine suspension are the paediatric products without that restriction, and they are listed in depth on the paediatric PCD franchise page.

Many distributors begin with a wet-cough syrup, a dry-cough syrup, a cold-and-flu tablet and montelukast-levocetirizine, then add junior packs, drops, anti-asthma tablets and the VENTILA inhalation line as paediatricians, ENT surgeons and chest physicians are converted.
