Description
Vonoprazan 20 mg tablet (as vonoprazan fumarate). Vonoprazan is a potassium-competitive acid blocker (P-CAB). It binds reversibly to the proton pump (H+/K+-ATPase) of the stomach's parietal cells and blocks it in competition with potassium, so acid secretion is suppressed. It acts without acid activation, is stable in acid and suppresses acid from the first dose, through day and night; its effect varies little with CYP2C19 genotype or meal timing. Randomised trials and meta-analyses have studied it for healing and maintenance of erosive oesophagitis and in dual or triple Helicobacter pylori regimens, including clarithromycin-resistant infection. Taken with or without food.
Side Effects
Diarrhoea, constipation, nausea, abdominal distension, headache, rash., Raised serum gastrin is expected with long-term use., Less common: raised liver enzymes, oedema., Rare: severe allergic or skin reactions, Clostridioides difficile-associated diarrhoea, low blood counts., Long-term strong acid suppression may lower absorption of vitamin B12, iron and magnesium., It reduces absorption of drugs that need stomach acid (atazanavir, rilpivirine, some kinase inhibitors), which should not be co-prescribed; strong CYP3A4 inducers and inhibitors alter its levels., Not for use with atazanavir or rilpivirine, or in known allergy to vonoprazan.
Indication
Healing and maintenance of healing of erosive oesophagitis , Gastro-oesophageal reflux disease and heartburn , Gastric and duodenal ulcer , Prevention of ulcer recurrence during long-term NSAID or low-dose aspirin therapy , Helicobacter pylori eradication with amoxicillin, with or without clarithromycin
