Vildalam M 500 tablets are an anti-diabetic combination of Vildagliptin 50 mg and Metformin 500 mg for adults with type 2 diabetes. Main uses: improves blood sugar control in adults with type 2 diabetes mellitus; used along with diet and exercise; helps lower blood sugar after meals and between meals; used when diet and exercise alone do not keep blood sugar in range.

Common side effects of Vildalam M 500 Tablet include diarrhoea; nausea; headache; upper respiratory tract infection; dizziness; flatulence. Get medical help at once if you notice lactic acidosis (metformin-associated; symptoms include malaise, muscle pain, breathing difficulty, sleepiness and abdominal pain); acute pancreatitis (severe abdominal pain; the tablet should be stopped if suspected); serious allergic reactions including anaphylaxis and angioedema.

Vildagliptin acts as a DPP-4 inhibitor, slowing the breakdown of incretin hormones that rise after eating, which helps the body release insulin when it is needed. Metformin, the biguanide component, works mainly at the liver to reduce sugar production and also improves how the body responds to insulin. Acting together, the two ingredients help bring blood sugar closer to the normal range through the day.

Must not be used in severe renal impairment (eGFR below 30 mL/min/1.73 m2). Must not be used in acute or chronic metabolic acidosis, including diabetic ketoacidosis. Must not be used if there is a history of a serious allergy to vildagliptin, metformin or any ingredient of the tablet. Must not be used for type 1 diabetes mellitus. Check with a doctor first if kidney function is reduced, if liver function is reduced or liver enzymes are raised (vildagliptin should be avoided in hepatic impairment or when liver enzymes are markedly elevated, and liver function should be assessed before treatment and monitored during treatment), if you drink alcohol heavily, if you are pregnant or breastfeeding, or before surgery or scans with contrast dye.
