What vitamin D3 is

Vitamin D3 (cholecalciferol) is the form of vitamin D that your skin makes when UV-B sunlight converts 7-dehydrocholesterol, and it is also the form found mainly in animal-source foods. The liver and then the kidney activate it before the body can use it.

Vitamin D2 (ergocalciferol) comes from plants and mushrooms. The IAP 2021 guideline notes that when large intermittent doses are used, D3 is more effective than D2.

According to the NHS, vitamin D helps the body regulate calcium and phosphate, which are needed for healthy bones, teeth and muscles. The unit conversion used everywhere in India is 1 microgram = 40 IU, so 400 IU is 10 micrograms.

How much vitamin D you need

Daily vitamin D needs are modest: the ICMR-NIN 2020 recommended dietary allowance (RDA) for adults in India is 600 IU, with an estimated average requirement (EAR) of 400 IU.

Reference intakes by age

The IOM/NASEM 2011 values, which underlie the US National Institutes of Health figures, set the RDA at 400 IU for infants aged 0–12 months, 600 IU from 1 to 70 years, and 800 IU above 70 years. The NHS gives 10 micrograms (400 IU) a day for adults and children from 1 year, and 8.5–10 micrograms for babies.

The IAP 2021 guideline recommends 400 IU a day for all infants through the first year, and states that higher doses give no extra skeletal benefit. It does not recommend routine supplementation in older children and adolescents.

Upper limits

  • IOM tolerable upper intake levels: 1,000 IU (0–6 months), 1,500 IU (6–12 months), 2,500 IU (1–3 years), 3,000 IU (4–8 years) and 4,000 IU (9 years and above).
  • NHS UK limits: 1,000 IU under 12 months, 2,000 IU for 1–10 years, 4,000 IU for over 11 years and adults.
  • IAP 2021 warns that daily doses of 1,600 IU or more can cause toxicity in children.

These are reference limits for the general population, not treatment doses. Treatment doses for deficiency are higher and are set by a doctor.

Product forms in India: drops, shots, sachets, softgels and injections

Indian pharmacies stock vitamin D3 in five practical forms, and the strength per millilitre differs between products, so always use the dropper or measuring device supplied with that product.

Infant and paediatric drops

Drops are listed at 400 IU per 0.5 ml or 800 IU per ml. Because the two strengths are not interchangeable millilitre for millilitre, the pack's own dropper and label instructions must be used, and a paediatrician should confirm the amount.

Oral solution "shots"

The liquid sold as a vitamin D3 shot is an oral solution of 60,000 IU in 5 ml, taken by mouth. It is taken on the schedule the prescriber writes.

Sachets and granules

Sachets contain 60,000 IU of cholecalciferol per 1 g. Follow the pack for how to take the granules, and the prescriber for how often.

Softgels and chewable tablets

Check the strength

Softgels are sold at 60,000 IU and also at 2,000 IU; chewable tablets at 60,000 IU are also sold. Check the strength on the pack: one 60,000 IU capsule is 15 times the 4,000 IU daily upper limit for adults (IOM, NHS), so it is not meant to be taken daily. Follow the prescribed schedule.

Injections

Injectable cholecalciferol comes as a 600,000 IU preparation. The IAP 2021 guideline states that oral dosing is preferred and that mega-dose injections (above 300,000 IU) are not recommended for children.

The IAP guideline also notes that oral vitamin D in India is packed in units up to 60,000 IU, whereas other countries commonly use 50,000 IU units. Indian products should therefore be read against Indian labels rather than foreign instructions.

Doses used for deficiency

Treatment schedules for vitamin D deficiency differ by guideline, age and country, and the information below is given only to explain what published sources say; your doctor decides the dose and duration for you.

Indian expert consensus 2025

The 2025 consensus of 41 endocrinologists, published in the Indian Journal of Endocrinology and Metabolism, describes treatment of adults with deficiency as 60,000 IU once a week for 12 weeks followed by a monthly maintenance dose. For insufficiency it describes 60,000 IU weekly for 8 weeks and then monthly. The consensus does not recommend a single loading dose of 1,00,000 IU or more in India.

IAP 2021 for children

For children, the IAP guideline keeps daily dosing as the first-line approach: 2,000 IU a day under 1 year and 3,000 IU a day above 1 year, for 12 weeks. An alternative for children over 12 months is 60,000 IU every two weeks for 5 doses. Oral treatment is preferred to injections.

United Kingdom product information

The UK summary of product characteristics for a cholecalciferol product describes adult deficiency treatment as 50,000 IU a week for 6 weeks, or 3,000–4,000 IU a day for 10–12 weeks.

Endocrine Society 2024

The 2024 clinical guideline from the Endocrine Society, co-sponsored by the Endocrine Society of India, states that for adults aged 50 and over who need vitamin D, daily lower doses are preferred over non-daily high doses. It replaces the society's 2011 guideline.

Whatever the form, follow the label and your doctor's instructions, and do not change a prescribed schedule on your own.

Testing for vitamin D deficiency

There is no single agreed threshold for vitamin D deficiency, and the leading guidelines do not agree with each other on either the cut-off or on who should be tested.

National data show that low levels are common. The CNNS 2019 survey, reported through the ICMR-NIN Dietary Guidelines 2024, found vitamin D deficiency in 13.7% of children aged 1–4, 18.2% of those aged 5–9 and 23.9% of those aged 10–19.

Because the guidelines differ, the decision to test, and what a result means for you, should come from a doctor.

Safety and toxicity

Vitamin D is fat-soluble, and repeated high doses can build up in the body.

Signs of too much

The UK product information lists nausea, vomiting, excessive thirst and urination, loss of appetite, weakness, constipation and drowsiness as signs of excess; chronic overdose can cause calcification of blood vessels and organs.

Too much by accident

In the UK, the NHS tells people to call its 111 helpline if, by accident, an adult takes more than 4,000 IU in a day, a child aged 1–10 more than 2,000 IU, or a baby under 12 months more than 1,000 IU. In India, contact a doctor or hospital if you or a child has taken too much.

Who must be careful

  • People with high blood calcium, kidney stones, severe kidney disease or sarcoidosis should take care or avoid high-dose vitamin D.
  • Interactions are listed with thiazide diuretics (which can raise blood calcium), digoxin, steroids, phenytoin and barbiturates.
  • Anyone on these medicines should tell the prescribing doctor before starting a cholecalciferol supplement.

Evidence from India

Indian reports point mainly to injections as the risk factor. A study of 32 patients with vitamin D toxicity found that all had received multiple intramuscular injections, with a median cumulative dose of 42 lakh IU. A separate laboratory audit found rising very high vitamin D levels between 2011 and 2016 and discouraged unmonitored high-dose and injected dosing.

Guidelines prefer oral treatment, and an injection should be given only on a doctor's prescription.

Rules and prices in India

Vitamin D3 is regulated in India both as a drug and, in some packs, as a dietary supplement, which is why labelling and pricing vary.

  • The National List of Essential Medicines 2022 (section 26.4) lists cholecalciferol as solid oral 1,000 IU, solid oral 60,000 IU and oral liquid 400 IU/ml.
  • The NPPA has fixed a ceiling price for the cholecalciferol 1,000 IU sachet or granules at ₹23.14 per gram excluding GST (S.O. 939(E), 28 February 2024).
  • Some 60,000 IU products carry "Prescription required; NPPA regulated" on pharmacy listings, while others are labelled "Dietary supplement. Not for medicinal use". Read the pack itself before buying or dispensing.
