People look at Bihar because the state is large in population and has a documented network of health facilities and an active medicine trade recorded on the state drug licensing portal. The figures below are from official government sources.
Bihar's population at Census 2011 was 10,40,99,452 (about 10.41 crore), which was 8.60% of India's population. The state portal records 38 districts, 101 sub-divisions, 534 community development blocks, 8,406 panchayats, 45,103 revenue villages and 199 towns.
Nearly 89% of the population lived in rural areas in 2011, and the state portal puts the urbanisation rate at 11.29%. A forward estimate from the National Commission on Population (Ministry of Health and Family Welfare, November 2019 report) projects Bihar at about 13.23 crore people in 2026.

At the primary level, Bihar had 1,994 primary health centres as on 31 March 2019 (1,899 rural and 95 urban), as reported by PIB from the Rural Health Statistics 2019. A PIB annexure based on the AAM Portal shows 10,334 Ayushman Arogya Mandirs operationalised in Bihar as of 29 July 2024.
For the medicine trade itself, the state's XLN drug licensing portal showed 84,012 sales firms and 1,69,449 licences on 24 October 2026.
Bihar Medical Services & Infrastructure Corporation Ltd (BMSICL) describes itself as the sole procurement and distribution agency of drugs and equipment for all establishments under the Department of Health, Government of Bihar. It publishes a state Essential Drug List (EDL 2018) and runs rate-contract drug tenders, with warehouses recorded at Fatuha in Patna, Narayanpur Anant in Muzaffarpur, Taranagar in Purnia and Raghopur in Bihta.
Vibcare appoints one franchise partner per district in Bihar, with monopoly rights confirmed in writing, and availability for your district is confirmed when you enquire. Bihar has 38 districts, and the state government's Bihar Vikas Mission groups them into nine divisions as listed below.
Names and spellings follow the Government of Bihar state portal's list of districts and the Bihar Vikas Mission's division-wise grouping. At Census 2011 the most populous district was Patna (58,38,465) and the least populous was Sheikhpura (6,36,342).
In Bihar the wholesale drug licence is issued by the Directorate of Drugs Control Administration under the Health Department, and the statutory route is an application in Form 19 followed by a licence in Form 20B or Form 21B. The requirements below come from the Drugs and Cosmetics Rules, 1945, which apply in the same form across India.
The State Licensing Authority is the Directorate of Drugs Control Administration, headed by the State Drugs Controller. As listed by CDSCO (list as on 24 April 2026), the office is at 6th Floor, Swasthya Bhawan, Sheikhpura, Patna, Bihar 800014, with the email [email protected].
Bihar's licensing portal is its own state system, XLN (Xtended Licensing Node) at xln.bihar.gov.in.
Applications run through the XLN portal, whose public menu covers application status, licence lookups for retailers and wholesalers, registered pharmacists and cancellation or suspension entries. The portal's live counters on 24 October 2026 showed 77,411 online applications counted, of which 70,769 were granted and 897 rejected.
Bihar uses its own XLN portal rather than the central ONDLS system; the directorate office can tell you which documents to upload.
Rule 59(2) of the Drugs and Cosmetics Rules, 1945 sets the application fee at ₹1,500 for a licence in Form 19 for drugs other than Schedule X. Rule 61 then issues the wholesale licence in Form 20B (drugs other than Schedules C, C(1) and X) or Form 21B (Schedule C and C(1) drugs, excluding Schedule X, which has its own licence), and Rule 62 requires a separate application and a separate licence for each place of sale.
Rule 64 sets the conditions for grant. The premises must be adequate and equipped with proper storage accommodation for preserving the properties of the drugs, and the wholesale premises must be not less than 10 square metres. If retail (Form 20/21) and wholesale (Form 20B/21B) licences are both sought from the same premises, the area must be not less than 15 square metres.
The premises must be in the charge of a competent person who is a Registered Pharmacist, or a matriculate with four years' experience in dealing with sale of drugs, or a degree holder of a recognised university with one year's experience in dealing with drugs. If you are unhappy with the licensing authority's order, Rule 64(3) allows an appeal to the State Government within 30 days of receiving it.
A licence in Form 20B or 21B remains valid perpetually provided a licence retention fee is paid before the end of every five years from the date of issue, under Rule 63 as substituted by G.S.R. 1337(E) of 27 October 2017. The retention fee equals the fee for grant, so ₹1,500; late payment adds 2% of the licence fee per month or part thereof for up to six months, after which the licence is deemed cancelled.
Rule 65B requires the licensing authority to have every licensed premises inspected not less than once in three years, or as needed on a risk basis. Day to day, a wholesale supply is made against a cash or credit memo bearing the licensee's name, address and licence number, and showing the buyer's name, address and sale-licence number, or in the case of a government authority, institution or registered medical practitioner, that recipient's name and address. The memo also records the drug, quantity, batch and manufacturer, and is signed by the competent person; carbon copies are kept for three years.
GST registration is separate from the drug licence, and Vibcare asks partners to hold it before starting. Under the CGST Act 2017, registration is compulsory whatever the turnover for anyone making inter-State taxable supplies; for a supplier of goods only, the threshold is ₹40 lakh (Notification No. 10/2019-Central Tax).

Apply to the Directorate of Drugs Control Administration, Bihar, which is the State Licensing Authority, through the state's XLN online portal. The statutory application is Form 19 with a fee of ₹1,500 under Rule 59(2), and the wholesale licence is then issued in Form 20B, or Form 21B for Schedule C and C(1) drugs (excluding Schedule X).
You will need premises of at least 10 square metres with proper storage, and a competent person in charge — a Registered Pharmacist, or a matriculate with four years' experience in dealing with sale of drugs, or a graduate with one year's experience. The directorate office confirms the documents to upload.
The Directorate of Drugs Control Administration is at 6th Floor, Swasthya Bhawan, Sheikhpura, Patna, Bihar 800014, as listed by CDSCO in its list of State Licensing Authorities as on 24 April 2026. The email on that list is [email protected].
The online licensing portal is XLN at xln.bihar.gov.in. The state drug buyer BMSICL also has its office in Swasthya Bhawan at Sheikhpura, Patna 800014.
Bihar has 38 districts, grouped into nine divisions — Patna, Saran, Tirhut, Purnia, Bhagalpur, Darbhanga, Kosi, Magadh and Munger — with Patna as the capital. At Census 2011 the most populous district was Patna and the least populous was Sheikhpura.
Bihar's scale is documented: 10.41 crore people at Census 2011 (8.60% of India), projected at about 13.23 crore in 2026 by the National Commission on Population, spread over 38 districts, 534 CD blocks, 45,103 revenue villages and 199 towns. The care and trade network recorded officially includes 21 medical colleges with 2,665 MBBS seats (2023), 1,994 primary health centres (31 March 2019) and 10,334 Ayushman Arogya Mandirs (July 2024).
On the trade side, the state drug licensing portal showed 84,012 sales firms and 1,69,449 licences on 24 October 2026, and BMSICL is the sole procurement and distribution agency for drugs and equipment for state health establishments. Tell us your district to check whether it is open.
The law tests the activity, not the business name: section 18(c) of the Drugs and Cosmetics Act, 1940 bars anyone from selling, stocking, exhibiting or offering for sale, or distributing a drug without a licence. So if you stock or sell medicines, a licence is required.
The only named exemption in the Rules is for the free distribution of samples of medicines by travelling agents and agents of licensed manufacturers to the medical profession and institutions. The Act and Rules do not define "PCD" at all.
Wholesale dealing means sale to a person for the purpose of selling again, and includes sale to hospitals, dispensaries and medical, educational or research institutions; selling to chemists for resale is therefore wholesale. The wholesale licence is Form 20B for drugs other than Schedules C, C(1) and X, or Form 21B for Schedule C and C(1) drugs (excluding Schedule X).
Retail sale to patients is licensed in Form 20 or Form 21. Which form you need depends on whom you supply, so decide your supply pattern before applying.
The premises must be in the charge of a competent person who is a Registered Pharmacist, or has passed matriculation with four years' experience in dealing with sale of drugs, or holds a degree of a recognised university with one year's experience in dealing with drugs. The licensing authority must also be satisfied that the person is competent to supervise and control the sale, distribution and preservation of drugs.
Separately, the premises must be adequate with proper storage accommodation, and not less than 10 square metres for a wholesale licence.
The minimum first order with Vibcare is ₹20,000, and orders are dispatched the same day. To start a Vibcare franchise you need a wholesale drug licence and GST registration; the statutory application fee for a sale licence in Form 19 is ₹1,500 under the Drugs and Cosmetics Rules.
